| Researchers, professors, J-1 Exchange Program
Students, and their dependents need the DS 2019 Form
in order to: obtain the J-1/J-2 Visa, to enter the U.S.
and to stay in the U.S. while engaged in research, teaching,
special study programs or events. |
| The DS 2019 Form is issued, after
the Queens College faculty host reads the cover
letter to QC faculty and prospective
J-1's and then submits the J-1
International Exchange Visitor Application Form to
ISSO. |
| Steps to Obtain the
DS 2019: |
| 1. |
A Queens College faculty host completes the Queens
College J-1 Application
Form from The International
Students & Scholars
Office(ISSO). |
| 2. |
The Queens College faculty host and prospective J-1
complete the Queens College J-1
Application Form together. |
| 3. |
Queens College faculty host submits the J-1 Application
to the Provost for approval. The Provost forwards
it to ISSO. |
| 4. |
ISSO reviews the Application, and then issues DS 2019
to the faculty host. |
| 5. |
The faculty host mails DS 2019 to prospective J-1/J-2
(The DS 2019 can not be e-mailed nor faxed.) |
| 6. |
The prospective J-1 takes the DS 2019, passport, and
SEVIS I 901 receipt to the nearest U.S. Consulate to
apply for the J-1 Visa. |
